# Chronic Pancreatitis

## Learning Objectives

- Define chronic pancreatitis as a progressive fibro-inflammatory disease with irreversible structural injury and variable functional consequences.

## Definition and core disease concept

Chronic pancreatitis is a **pathologic fibro-inflammatory syndrome of the pancreas** that can produce permanent structural change and progressive loss of exocrine or endocrine function. The diagnosis is not synonymous with one symptom, one laboratory value, or one imaging finding. It is an integrated judgment that combines:

- A compatible clinical syndrome.
- Exposure or predisposition risk.
- Structural pancreatic changes on imaging.
- Evidence of exocrine or endocrine dysfunction when present.
- Exclusion of important mimics, especially pancreatic cancer, autoimmune pancreatitis, peptic disease, biliary disease, and functional or neuropathic pain syndromes.

The central disease sequence is **injury → inflammation → fibrosis and duct distortion → pain and functional loss**. Some patients are pain-predominant early in the disease, while others first present with maldigestion, weight loss, diabetes, recurrent acute pancreatitis, or a complication. A normal lipase does not exclude established disease, and a diagnosis of chronic pancreatitis does not automatically explain every new symptom.

- It is a chronic disease process, not simply “recurrent pancreatitis pain.”
- It can exist with minimal or no steatorrhea early in the course because the pancreas has substantial functional reserve.
- It can cause recurrent acute flares, but recurrent acute pancreatitis and chronic pancreatitis are not interchangeable diagnoses.
- New jaundice, progressive weight loss, a new mass, or abrupt diabetes require reassessment rather than automatic attribution to chronic pancreatitis.
- The evidence base is uneven: imaging and ductal interventions have guideline and trial data, whereas some natural-history and lifestyle recommendations rely on observational evidence or expert consensus.

## Etiologies and risk factors

Use a broad etiologic assessment instead of assuming alcohol is the only cause. A structured approach helps identify reversible injury, informs genetic counseling, and prevents missed autoimmune or obstructive disease.

Common categories include:

- **Toxic-metabolic:** alcohol exposure, tobacco use, severe hypertriglyceridemia, hypercalcemia, and selected medications or toxins.
- **Idiopathic:** no cause is found despite an appropriate evaluation.
- **Genetic:** PRSS1, SPINK1, CFTR, CTRC, and other pancreatitis-associated variants; genetic testing is especially relevant in younger patients, recurrent acute pancreatitis, familial disease, or unclear etiology.
- **Autoimmune:** type 1 or type 2 autoimmune pancreatitis, often suggested by imaging, serum IgG4, other organ involvement, or steroid responsiveness—but not diagnosed by IgG4 alone.
- **Recurrent acute pancreatitis:** repeated acute injury can evolve into chronic structural disease.
- **Obstructive:** pancreatic duct stones or strictures, pancreas divisum in selected clinical contexts, ductal anomalies, prior trauma, or a tumor causing upstream obstruction.
- **Hereditary or syndromic disease:** consider cystic fibrosis-related disease and other inherited disorders when the history is suggestive.

Key history items when evaluating risk factors:

- Ask about amount, frequency, duration, and pattern of alcohol exposure.
- Ask about cigarettes, vaping, smokeless tobacco, and secondhand exposure.
- Document family history of pancreatitis, pancreatic cancer, cystic fibrosis, or unexplained recurrent abdominal pain.
- Review prior acute pancreatitis episodes, gallstone history, hypertriglyceridemia, hypercalcemia, and relevant medications.
- Offer evidence-based treatment for alcohol use disorder and tobacco cessation; ACG makes strong recommendations for cessation, but the guideline grades the supporting evidence as very low because randomized cessation trials in chronic pancreatitis are lacking.

**Guideline:** The 2020 ACG Clinical Guideline recommends alcohol cessation and smoking cessation for patients with chronic pancreatitis.

**Evidence strength:** These are strong recommendations, but the guideline notes very low-quality direct evidence for changing the natural history of chronic pancreatitis.

**Clinical meaning:** The low evidence grade does not make cessation unimportant. It means the exact effect size is uncertain. Treat cessation as disease-directed care, while also providing medications, counseling, and addiction-treatment referral rather than presenting advice alone as treatment.

## Pathophysiology: why pain and organ failure develop

Repeated or persistent pancreatic injury activates inflammatory and repair pathways. Stellate cells and other stromal cells produce extracellular matrix, leading to fibrosis. Fibrosis narrows and distorts ducts, while protein plugs, stones, strictures, and altered duct pressures can further impair drainage.

Pain is biologically heterogeneous. Possible contributors include:

- Ductal hypertension from obstruction or stones.
- Inflammatory mass effect or complications such as pseudocyst.
- Ischemia and local inflammation.
- Peripheral nerve injury and perineural inflammation.
- Central sensitization and altered pain processing.
- Visceral hypersensitivity, fear-avoidance, depression, sleep disturbance, and opioid-induced hyperalgesia.
- Non-pancreatic causes that coexist with or mimic pancreatic pain.

 u Exocrine insufficiency /u  develops when acinar enzyme delivery becomes inadequate for the patient’s nutritional needs. The consequences can include impaired digestion of fat and protein, altered absorption of fat-soluble vitamins, weight loss, sarcopenia, and bone disease. Clinically obvious steatorrhea often occurs late because the pancreas has considerable reserve.

 u Endocrine injury /u  causes **pancreatogenic diabetes, often called type 3c diabetes**. Loss of insulin-producing beta cells can coexist with loss of glucagon-producing alpha cells, impaired incretin responses, malnutrition, and inconsistent food intake. The result can be a higher risk of both hyperglycemia and difficult-to-recognize hypoglycemia.

## Clinical features

Symptoms vary with disease stage, etiology, anatomy, and the degree of exocrine or endocrine failure.

Typical features include:

- Chronic or recurrent epigastric pain, often radiating to the back.
- Pain triggered or worsened by meals, sometimes leading to food avoidance.
- Nausea, early satiety, bloating, or reduced intake.
- Recurrent episodes that resemble acute pancreatitis.
- Steatorrhea, oily stools, diarrhea, urgency, or excessive gas.
- Weight loss, low muscle mass, fatigue, or micronutrient deficiency.
- New or worsening diabetes, particularly with weight loss or pancreatic calcification.
- Jaundice or cholestatic liver tests from distal common-bile-duct obstruction.
- Vomiting or early satiety from duodenal or gastric outlet obstruction.
- Fever, persistent vomiting, or acute deterioration from infected or complicated fluid collections.

The physical examination may be surprisingly unrevealing. Look for:

- Low body mass index or loss of lean mass.
- Abdominal tenderness, guarding, or a palpable mass.
- Jaundice, dehydration, or orthostasis.
- Signs of diabetes or recurrent hypoglycemia.
- Bruising or bone pain suggesting nutritional deficiency.
- Evidence of alcohol withdrawal, intoxication, or tobacco dependence when relevant.

Serum amylase and lipase can be normal in established chronic pancreatitis because damaged acinar tissue may no longer release large amounts of enzyme. Use pancreatic enzymes when evaluating an acute episode or another acute abdominal process, but do not use a normal lipase to dismiss a compatible chronic pancreatitis syndrome. The diagnosis still requires clinical correlation and structural assessment.

## Complications and red flags

Complications may be structural, vascular, nutritional, metabolic, or malignant.

Important complications to actively assess for:

- Pancreatic pseudocyst or walled-off fluid collection.
- Pancreatic duct disruption or fistula.
- Biliary obstruction or cholangitis.
- Duodenal obstruction.
- Splenic vein thrombosis, portal hypertension, gastric varices, or bleeding.
- Exocrine pancreatic insufficiency and malnutrition.
- Osteopenia or osteoporosis.
- Pancreatogenic diabetes.
- Recurrent acute pancreatitis.
- Depression, anxiety, substance-use disorder, and chronic pain disability.

Red flags should trigger diagnostic reassessment rather than automatic escalation of analgesia:

- New or progressive jaundice.
- A new pancreatic or biliary mass.
- Rapid unexplained weight loss.
- Abrupt new diabetes or rapid worsening of glycemic control.
- A change in a longstanding pain pattern.
- Persistent vomiting, fever, gastrointestinal bleeding, or anemia.
- A focal duct cutoff, upstream atrophy, or a new dominant stricture.
- Atypical age or family history suggesting inherited disease or malignancy.

Routine cancer screening is not recommended for every patient with chronic pancreatitis solely because the diagnosis is present. Use individualized surveillance or high-risk pancreatic programs when a defined hereditary syndrome or strong familial risk warrants it, and investigate symptoms or imaging changes promptly.

## Diagnostic approach: establish structure, then assess function

Begin with the clinical syndrome, risk factors, prior episodes, and any features that require urgent evaluation for an alternative diagnosis.

### Step 1: Define the clinical question

Ask:

- Is the patient having chronic pancreatic-type pain, recurrent acute pancreatitis, maldigestion, diabetes, or a complication?
- Is there a reason to suspect a mass, autoimmune pancreatitis, biliary disease, peptic disease, or another mimic?
- Is the question diagnosis, staging, treatment planning, or evaluation of a new complication?
- What is the pretest probability based on symptoms, exposures, family history, and prior imaging?

### Step 2: Use cross-sectional imaging first

The ACG guideline recommends CT or MRI as first-line imaging for suspected chronic pancreatitis. Choose based on the clinical question, local expertise, radiation considerations, and the need to characterize ducts or fluid collections.

Look for:

- Parenchymal atrophy or heterogeneous gland texture.
- Pancreatic calcifications.
- Main-duct dilation or irregularity.
- Side-branch ectasia.
- Stones, strictures, or duct cutoff.
- Pseudocyst or other fluid collection.
- Inflammatory mass, biliary obstruction, or features concerning for malignancy.

MRI/MRCP can better characterize ductal anatomy and fluid collections without radiation. Pancreas-protocol CT is often readily available and useful for calcification, vascular anatomy, complications, and cancer assessment.

### Step 3: Add Endoscopic Ultrasound (EUS) selectively

Use EUS when CT or MRI/MRCP is nondiagnostic but suspicion remains, for early or minimal-change disease, or when a focal lesion requires close evaluation or tissue sampling. EUS findings can be nonspecific, especially when only mild changes are present, so interpret them alongside the clinical context.

If the diagnosis remains uncertain after cross-sectional imaging and EUS but suspicion is high, secretin-enhanced MRCP may help demonstrate subtle ductal abnormalities. It is not a universal first-line test.

**Evidence:** The guideline cites a systematic review and meta-analysis of 43 studies and 3,460 patients in which sensitivity estimates were approximately 81% for EUS, 78% for MRI, and 75% for CT, with overlapping confidence intervals and limitations related to inconsistent histologic reference standards.

**Clinical meaning:** Do not interpret the small numerical differences as proof that EUS should replace CT or MRI. Availability, invasiveness, the suspected complication, and the need to evaluate a mass should drive sequencing.

### Step 4: Evaluate exocrine pancreatic function

Fecal elastase is a practical initial test when exocrine pancreatic insufficiency is suspected:

- Less than 100 micrograms/g supports exocrine pancreatic insufficiency.
- 100–200 micrograms/g is indeterminate and needs clinical correlation.
- A watery stool can dilute the sample and create a falsely low result.
- A normal or borderline result should be interpreted with the clinical syndrome, nutrition, imaging, and alternative causes of diarrhea.

Assess nutritional consequences with:

- Weight trend, BMI, muscle mass, and dietary intake.
- Fat-soluble vitamins and other micronutrients when clinically indicated.
- Albumin or prealbumin only as contextual markers, not stand-alone measures of nutrition.
- Bone mineral density because chronic pancreatitis is associated with osteopenia and osteoporosis.

Do not use fecal elastase as the primary test to prove chronic pancreatitis. It helps phenotype exocrine function after or alongside the structural and clinical assessment.

### Step 5: Evaluate endocrine pancreatic function

Assess for pancreatogenic diabetes separately from exocrine pancreatic insufficiency:

- Measure fasting plasma glucose and HbA1c at regular intervals.
- Test earlier when there is weight loss, polyuria, polydipsia, recurrent hypoglycemia, or abrupt worsening of glycemic control.
- Consider oral glucose tolerance testing when fasting glucose and HbA1c are inconclusive but suspicion remains high.
- Review nutritional status, pancreatic imaging, and the timing of diabetes onset to distinguish pancreatogenic diabetes from unrelated type 2 diabetes.
- New-onset diabetes with weight loss, jaundice, a new duct cutoff, or a new mass requires evaluation for pancreatic malignancy.

## Exocrine pancreatic insufficiency, nutrition, and pancreatic enzyme replacement therapy (PERT)

Exocrine pancreatic insufficiency is a treatable consequence of chronic pancreatic disease, not a synonym for chronic pancreatitis. Loss of acinar tissue and reduced ductal bicarbonate secretion decrease the delivery and activity of digestive enzymes in the small intestine. Inadequate lipase activity reduces fat digestion and absorption, while impaired protease and amylase delivery can contribute to protein and carbohydrate maldigestion. The resulting steatorrhea, weight loss, micronutrient deficiency, sarcopenia, and low bone density are consequences of inadequate nutrient digestion and absorption. It can coexist with other causes of malabsorption, so persistent symptoms despite PERT deserve reassessment.

Fibrosis and acinar-cell loss reduce pancreatic enzyme secretion. Duct injury can also reduce bicarbonate-rich fluid, leaving the duodenal lumen less favorable for enzyme activity. Management principles:

- Refer for dietitian support when weight loss, sarcopenia, restrictive eating, or micronutrient deficiency is present.
- Avoid unnecessary fat restriction; the goal is adequate energy and protein intake with enzyme coverage.
- Give PERT during the meal, not long before or after it.
- AGA clinical practice advice commonly starts adults at least at 40,000 USP units of lipase with each meal and half that amount with snacks, then titrates to meal size, symptoms, weight, and nutritional response.
- Confirm adherence, correct timing, and adequate dosing before labeling PERT failure.
- Consider acid suppression when an appropriate PERT trial is not working and gastric acid is interfering with enzyme activity.
- Monitor stool quality, weight, fat-soluble vitamins, bone health, and patient-reported function.
- Treat documented deficiencies and avoid megadose supplementation without a clinical indication.

PERT is intended to treat maldigestion and malnutrition. ACG does not suggest pancreatic enzymes solely as a treatment for chronic pancreatitis pain because randomized evidence has not shown reliable pain benefit.

**Bottom line:** A patient with steatorrhea, weight loss, or nutritional deficiency needs a treatment-and-monitoring plan—not only a test result.

**Applicability limits:** This result addresses pain, not maldigestion. PERT remains indicated for documented exocrine pancreatic insufficiency and nutritional consequences.

**Bottom line:** Prescribe PERT to replace digestive function—not as a stand-alone analgesic.

**Outcome:** A 2014 Cochrane review of 12 RCTs and 585 participants found a small reduction in pain scores over 1–6 months, but most trials were small with high dropout rates. Later systematic review evidence found no significant overall pain reduction, illustrating uncertainty and heterogeneity.

**Applicability limits:** Antioxidant formulations, doses, follow-up, and bias risk varied substantially. This is not a reason to recommend megadose supplements routinely.

**Bottom line:** ACG allows consideration of antioxidants with the expectation of limited benefit, but the evidence is mixed; discuss uncertainty and avoid presenting them as disease-modifying therapy.

## Pancreatogenic diabetes

Type 3c diabetes can be difficult to manage because pancreatic destruction may reduce insulin and glucagon secretion at the same time. Malnutrition, variable food intake, and inconsistent digestion can further increase glycemic variability and hypoglycemia risk.

Loss of pancreatic islet tissue reduces insulin secretion and can also reduce glucagon-mediated counter-regulation during hypoglycemia. Exocrine insufficiency, irregular eating, alcohol exposure, and malnutrition alter glucose absorption and hepatic substrate availability. As a result, glucose may rise after inconsistent nutrient absorption and fall unpredictably when insulin is present but glucagon responses are impaired. Treatment therefore requires coordination of glucose-lowering therapy, nutrition, and PERT, with attention to hypoglycemia education.

Practical considerations include:

- Screen periodically with fasting glucose and HbA1c; evaluate unexplained weight loss or abrupt glycemic change promptly.
- Confirm that diabetes is not simply unrelated type 2 diabetes, especially when obesity, metabolic syndrome, or a strong family history is present.
- Coordinate diabetes treatment with nutrition and PERT because improved digestion can change glucose exposure.
- Use individualized therapy based on insulin deficiency, nutritional status, hypoglycemia risk, kidney function, and patient preferences.
- Consider insulin when insulin deficiency is advanced, hyperglycemia is severe, or the patient is undernourished.
- Educate about hypoglycemia because glucagon counter-regulation may be impaired.
- Involve endocrinology when control is unstable, recurrent hypoglycemia occurs, or the diagnosis is uncertain.

New-onset diabetes with weight loss, jaundice, a new duct cutoff, or a new mass is a red flag for pancreatic malignancy and should not be attributed to chronic pancreatitis without evaluation.

## Pain: guideline-based, mechanism-directed management

Pain management should be multidisciplinary and reassessed over time. The evaluation should distinguish active obstructive disease from neuropathic or centrally amplified pain and should look for complications or non-pancreatic causes.

Start with:

- Reassess for a new complication or alternate diagnosis.
- Ask about pain severity, timing, meal relationship, sleep, function, mood, and opioid use.
- Support alcohol and tobacco cessation.
- Use non-opioid analgesics when safe.
- Consider neuropathic pain agents when the pattern suggests neural sensitization.
- Address depression, anxiety, sleep disruption, food fear, and reduced activity.
- Use opioids cautiously and only after other reasonable options have been addressed.
- Refer early to a pain specialist when pain is disabling, opioid exposure is escalating, or the diagnosis is uncertain.

The ACG guideline provides several specific recommendations:

- Do not prescribe pancreatic enzymes solely to treat chronic pancreatitis pain; use them for documented exocrine pancreatic insufficiency and malnutrition.
- Antioxidant therapy may be considered, but the expected pain benefit is limited and the evidence is inconsistent.
- Celiac plexus block may be considered in selected patients, although the supporting evidence is very low quality.
- Reserve opioids for patients in whom other reasonable treatment options have been exhausted, with monitoring for dependence, overdose, constipation, and opioid-induced hyperalgesia.
- Consider surgery or endoscopic therapy when pain is associated with ductal obstruction or another treatable structural complication rather than escalating analgesics alone.

## Anatomy-driven intervention: endoscopy, lithotripsy, or surgery

- ERCP with pancreatic sphincterotomy, stone extraction, stricture dilation, or stenting.
- Extracorporeal shock-wave lithotripsy for selected obstructing stones.
- Surgical drainage procedures such as lateral pancreaticojejunostomy, or combined drainage and head-coring procedures such as Frey surgery.
- Resection when there is a suspicious or inflammatory mass, small-duct disease, or anatomy unsuitable for drainage.
- Total pancreatectomy with islet autotransplantation only in highly selected refractory cases at experienced centers.

The intervention should be selected according to the anatomy and the treatment goal:

- **Ductal stones:** ERCP and/or extracorporeal shock-wave lithotripsy may be used to clear obstructing stones in selected patients.
- **Dominant strictures:** endoscopic dilation and temporary stenting may be considered, with reassessment for persistent obstruction or recurrence.
- **Dilated obstructed duct with persistent pain:** surgical drainage provides a more durable option when endoscopic drainage is unsuccessful, incomplete, or likely to require repeated procedures.
- **Inflammatory mass or suspected malignancy:** tissue diagnosis and surgical oncology or pancreatic surgery review may be required; drainage alone may not address the underlying problem.
- **Small-duct disease or refractory pain without a targetable obstruction:** outcomes are less predictable, so multidisciplinary pain and pancreas evaluation is important before irreversible procedures.

ACG recommends surgical intervention over endoscopic therapy for long-term pain relief when first-line endoscopic drainage has been exhausted or unsuccessful. The decision depends on duct diameter, stone burden, strictures, inflammatory mass, prior procedures, exocrine/endocrine reserve, pain phenotype, surgical risk, and patient goals.

**Population:** 88 adults with painful chronic pancreatitis and a dilated pancreatic duct with obstructing stones or strictures.

**Intervention:** Early surgical drainage within 6 weeks of randomization.

**Comparator:** An endoscopy-first strategy with medical treatment, endoscopic therapy, lithotripsy when needed, and surgery if necessary.

**Outcome:** Over 18 months, the mean Izbicki pain score was lower with early surgery (37 versus 49; between-group difference −12, 95% CI −22 to −2). The early-surgery group also underwent fewer interventions; complications, pancreatic function, and quality-of-life differences were not clearly significant.

**Limitations:** The trial was relatively small, focused on selected obstructive disease, and cannot be generalized to every patient with chronic pancreatitis or to pain without a targetable ductal lesion.

**Bottom line:** When obstructive disease is clearly driving pain, discuss surgery early with an experienced pancreas team, particularly when endoscopic drainage is unsuccessful or repeated procedures are anticipated.

**Population:** Adults with painful chronic pancreatitis enrolled in three randomized trials comparing definitive endoscopic and surgical treatment, totaling 199 participants.

**Intervention:** Surgical drainage or other surgical treatment for painful obstructive chronic pancreatitis.

**Comparator:** Endoscopic drainage, stone therapy, or an endoscopy-first strategy.

**Outcome:** Surgery produced more pain relief at medium and long-term follow-up. Some form of pain relief occurred in approximately 72% versus 46% at medium term and 73% versus 47% at long term; complete relief was also more frequent after surgery.

**Applicability limits:** The evidence base is small, procedures and patient selection vary, and surgery has greater upfront invasiveness. The pooled results apply most directly to painful obstructive disease managed in experienced centers.

**Bottom line:** For persistent pain with a correctable ductal obstruction, surgery should be part of the initial discussion—not merely a last resort after years of repeated procedures.

## Integrated clinical case

A 52-year-old patient with a long history of tobacco use and prior alcohol-associated acute pancreatitis reports two years of postprandial epigastric pain radiating to the back. They have lost 9 kg, pass greasy stools, and recently developed diabetes. CT shows pancreatic calcifications, a dilated main pancreatic duct, and an intraductal stone. Fecal elastase is 72 micrograms/g.

Reason through the case in sequence:

- **Syndrome:** chronic pancreatic-type pain plus maldigestion and new endocrine dysfunction.
- **Etiology:** prior alcohol-associated injury and tobacco exposure are important, but still review family history, triglycerides, calcium, medications, and obstructive causes.
- **Diagnosis:** calcifications and duct dilation strongly support established chronic pancreatitis; fecal elastase supports exocrine pancreatic insufficiency.
- **Red flags:** new diabetes and weight loss require attention to the possibility of malignancy or another obstructing process; do not stop at the label of chronic pancreatitis.
- **Immediate management:** support alcohol and tobacco cessation, assess nutrition and bone health, start PERT with meals and snacks, evaluate diabetes, and create a structured pain plan.
- **Anatomy-driven decision:** persistent pain with a dilated duct and obstructing stone warrants discussion with therapeutic endoscopy and pancreatic surgery. The RCT and meta-analysis evidence supports early surgical discussion, while the final choice depends on anatomy, expertise, risk, and patient preferences.
- **Follow-up:** reassess pain function, weight, stool quality, glycemic control, micronutrients, bone health, and new obstructive or malignant features.

## Summary

- Chronic pancreatitis is irreversible fibro-inflammatory remodeling with variable pain, exocrine insufficiency, diabetes, and structural complications.
- Diagnose the disease by integrating clinical context and risk factors with CT or MRI/MRCP; use EUS or secretin-enhanced MRCP selectively when uncertainty remains.
- Treat the patient’s consequences: stop ongoing injury, improve nutrition, use PERT for exocrine insufficiency, monitor bone and endocrine health, and manage pain with a mechanism-based plan.
- Normal serum lipase does not rule out established chronic pancreatitis, and chronic pancreatitis does not explain every new symptom.
- Ductal obstruction changes the management conversation. Endoscopy, lithotripsy, and surgery should be discussed by an experienced multidisciplinary team, with RCTs and meta-analyses favoring durable surgical pain control in selected obstructive disease.
- Evidence quality is not uniform: diagnostic sequencing, PERT practice, and surgery-versus-endoscopy decisions have different levels and types of support. State those limits when counseling patients.
